COVID-19: Special Circumstance Requests

While the university is implementing safety practices and guidance for campus operations, special circumstances related to disability accommodations, safety and COVID testing may arise that require additional review and guidance. 

Please submit your exception request in the form below. Additional documentation may be requested to support your request, but should not be attached or hyperlinked in this form.  Special Circumstance Request submissions are a collaborative process between faculty/staff, supervisors, and local division/department HR representatives. As part of this process, your supervisor may be contacted directly for further information. If you have not discussed this request with your supervisor, you are strongly encouraged to do so, and share the submitted information.

Representatives from Environmental Health and Public Safety, University Human Resource, Disability Resource Office, and the Office for Institutional Equity and Diversity can review and assist with resolving individual requests.  Upon completion of the form, the information will be routed to the appropriate reviewers. You will receive an email with your submission. Generally, you will receive follow-up from reviewers within five business days of your request.
